Please read these Terms and Conditions carefully before using the ConciergeRX website or services. By accessing our website, submitting an assessment, booking a consultation, or using our services, you agree to be bound by these Terms. If you do not agree, please do not use our website or services.

1. About ConciergeRX

Concierge RX, LLC ("ConciergeRX," "we," "us," or "our") provides physician-supervised longevity, wellness, and health optimization services primarily through telehealth.

Our services may include advanced blood analysis, biomarker reviews, home phlebotomy coordination, physician consultations, personalized wellness protocols, health coaching, ongoing monitoring, and access to medications or supplements when determined clinically appropriate by a licensed provider.

Our website provides information about our programs and allows eligible individuals to request assessments, consultations, and related services.

2. Medical Disclaimer

The general content available on our website is provided for informational and educational purposes only. It is not intended to replace professional medical advice, diagnosis, or treatment.

Clinical services offered through ConciergeRX are provided or supervised by licensed healthcare professionals. No provider-patient relationship is created simply by visiting the website, submitting a form, joining a waitlist, or requesting information.

A provider-patient relationship may only be established after completion of the required intake process, consent forms, identity and location verification, and consultation with an appropriately licensed healthcare provider.

Always consult a qualified healthcare provider regarding any medical condition, symptoms, medications, or treatment decisions.

If you are experiencing a medical emergency, call 911 or visit the nearest emergency room immediately.

3. Eligibility

By using our website or services, you represent that:

  • You are at least 18 years old.
  • You have the legal capacity to enter into a binding agreement.
  • You will provide accurate and complete information.
  • You are physically located in a state where the applicable provider is licensed.
  • You will use our website and services only for lawful purposes.

ConciergeRX services may not be available in every state. We reserve the right to decline services if you do not meet legal, geographic, or clinical eligibility requirements.

4. Appointments and Consultations

a. Booking

Appointments may be requested through our website, scheduling platform, telephone, email, or another approved method.

Submitting a booking request does not guarantee that an appointment or service will be provided. We reserve the right to decline, cancel, or reschedule appointments based on provider availability, eligibility, safety, or clinical considerations.

b. Cancellations and No-Shows

Cancellation, rescheduling, deposit, and no-show requirements will be disclosed during booking or in your appointment confirmation.

Appointments cancelled without sufficient notice or missed without notification may result in a cancellation or no-show fee.

c. Appointment Preparation

You may be required to complete intake forms, provide identification, disclose your current location, submit medical records, or complete laboratory testing before your consultation.

Late arrivals may result in a shortened appointment, cancellation, or rescheduling.

5. Telehealth Services

Some ConciergeRX services are provided through video calls, telephone calls, electronic forms, secure messaging, or other telehealth technologies.

By participating in telehealth services, you understand that:

  • Telehealth may have limitations compared with an in-person examination.
  • Technical issues may interrupt or delay a consultation.
  • A provider may require additional testing or an in-person evaluation.
  • You are responsible for using a private location and secure internet connection.
  • You must not record consultations without prior permission.

A separate telehealth consent form may be required before services are provided.

6. Payments and Fees

Payment may be required before a consultation, laboratory service, assessment, membership, product, or other service is provided.

By providing payment information, you authorize ConciergeRX and its payment processors to charge the applicable fees.

Prices may vary depending on your selected program, physician recommendations, laboratory requirements, medications, supplements, and other services. Prices may change for future services without prior notice.

You are responsible for any fees charged by your bank, credit card provider, financing provider, or payment processor.

Refunds, where available, will be governed by the refund or cancellation policy disclosed at the time of purchase.

7. Insurance

Unless expressly stated otherwise, ConciergeRX services may not be covered by health insurance, Medicare, or Medicaid.

You are responsible for confirming whether your insurance provider offers reimbursement for consultations, laboratory testing, prescriptions, or other services.

ConciergeRX does not guarantee insurance coverage or reimbursement. You remain responsible for all applicable fees regardless of whether your insurance provider reimburses you.

8. Health Information and Informed Consent

Before receiving services, you may be required to:

  • Complete a health and medical-history questionnaire.
  • Disclose current medications, supplements, allergies, and health conditions.
  • Provide laboratory results or other relevant records.
  • Review and sign informed-consent documents.
  • Confirm your identity and physical location.

You represent that all information you provide is truthful, current, and complete.

Incomplete or inaccurate information may affect the safety and effectiveness of your care. ConciergeRX is not responsible for outcomes caused by information that you intentionally fail to disclose or provide inaccurately.

9. Laboratory Testing and Home Phlebotomy

ConciergeRX may coordinate laboratory testing, home blood collection, specimen collection, or related services through independent laboratories and licensed service providers.

You are responsible for following all preparation, collection, storage, and shipping instructions.

Testing or processing may be delayed due to insufficient samples, collection errors, shipping delays, laboratory issues, weather, or circumstances outside our control.

Laboratory results should not be used for self-diagnosis or independent changes to your medications or treatment plan. Results should be reviewed with a qualified healthcare provider.

10. Prescriptions and Clinical Decisions

All clinical decisions are made by licensed healthcare providers using their independent professional judgment.

Completing an assessment or consultation does not guarantee that you will receive a prescription, medication, supplement, laboratory order, or particular treatment recommendation.

A provider may determine that:

  • A requested service is not clinically appropriate.
  • Additional testing or records are required.
  • An in-person evaluation is necessary.
  • You should seek care from another healthcare provider or specialist.

Any prescription medication must be used only as directed by the prescribing provider and dispensing pharmacy.

11. Compounded and Off-Label Medications

Certain medications recommended through ConciergeRX may be compounded by licensed pharmacies or prescribed for an off-label use when a licensed provider determines that doing so is clinically appropriate.

Compounded medications are not reviewed or approved by the FDA in the same manner as commercially manufactured, FDA-approved medications.

The prescribing provider or pharmacy will provide applicable information regarding potential benefits, risks, alternatives, dosage, and proper use.

12. No Guarantees of Results

Individual results vary based on many factors, including age, medical history, genetics, lifestyle, nutrition, physical activity, sleep, health conditions, medication use, and adherence to provider recommendations.

ConciergeRX does not guarantee any specific health, wellness, performance, weight, longevity, laboratory, or treatment outcome.

Testimonials, reviews, and case studies represent individual experiences and are not necessarily typical. They do not guarantee that you will achieve the same or similar results.

13. Third-Party Providers

ConciergeRX may use or coordinate services with independent laboratories, pharmacies, phlebotomists, technology providers, scheduling platforms, payment processors, delivery companies, meal providers, or other third parties.

These providers may have their own terms, privacy policies, fees, processing times, shipping policies, and eligibility requirements.

ConciergeRX is not responsible for the actions, availability, delays, errors, or services of independent third-party providers where those matters are outside our reasonable control.

14. Intellectual Property

All content available on the ConciergeRX website, including text, logos, graphics, videos, images, designs, questionnaires, software, educational materials, and branding, is owned by or licensed to ConciergeRX and is protected by applicable intellectual-property laws.

You may use website content only for personal and non-commercial purposes.

  • Scrape, crawl, or collect website data using automated tools.
  • Impersonate ConciergeRX, its providers, employees, or another person.
  • Interfere with the security or proper operation of the website.
  • Use prescriptions, services, or products for resale or for another person.

We may suspend or terminate access when these Terms are violated.

16. Privacy and Electronic Communications

Our collection and use of personal information are governed by our Privacy Policy and any applicable Notice of Privacy Practices.

By providing your contact information, you consent to receive appointment reminders, intake requests, service updates, billing notices, testing instructions, and other service-related communications by email, telephone, text message, secure messaging, or patient portal.

Standard message and data rates may apply.

You may opt out of marketing communications, but you may still receive necessary administrative, transactional, or care-related communications.

17. Limitation of Liability

To the fullest extent permitted by applicable law, ConciergeRX, its owners, employees, providers, contractors, and affiliates will not be liable for indirect, incidental, special, consequential, or punitive damages arising from:

  • Your use of or inability to use the website.
  • Reliance on general website content.
  • Website interruptions or technical errors.
  • Unauthorized access resulting from your failure to protect your account.
  • Third-party laboratory, pharmacy, shipping, payment, or technology services.
  • Delays or circumstances outside our reasonable control.

Nothing in these Terms limits any liability that cannot legally be limited under applicable law or applicable professional standards of care.

18. Indemnification

To the extent permitted by law, you agree to indemnify and hold harmless ConciergeRX and its owners, employees, contractors, providers, and affiliates from claims, damages, liabilities, and reasonable legal expenses resulting from:

  • Your unlawful use of the website.
  • Your violation of these Terms.
  • Your intentional submission of false or misleading information.
  • Your violation of another person's rights.

19. Governing Law

These Terms are governed by the laws of the State of Florida, without regard to its conflict-of-law provisions.

The parties agree to attempt to resolve disputes through good-faith communication before starting formal legal proceedings.

Any legal proceedings will be brought in an appropriate state or federal court located in Florida, unless otherwise required by applicable law or a separate written agreement.

20. Changes to These Terms

We may update or modify these Terms at any time.

Changes will become effective when posted on our website with an updated effective or "Last Updated" date.

Your continued use of the website or services after updated Terms are posted constitutes acceptance of the revised Terms.

21. Severability

If any provision of these Terms is found to be invalid or unenforceable, that provision will be modified or removed to the minimum extent necessary.

The remaining provisions will continue in full force and effect.

22. Entire Agreement

These Terms, together with our Privacy Policy, applicable consent forms, membership terms, payment policies, and service agreements, constitute the entire agreement regarding your use of the website and services.

Where a separate clinical or service agreement contains more specific terms, that agreement will apply to the relevant service.
