Appointments cancelled without sufficient notice or missed without notification may result in a cancellation or no-show fee.

Late arrivals may result in a shortened appointment, cancellation, or rescheduling.

7. Insurance

Unless expressly stated otherwise, ConciergeRX services may not be covered by health insurance, Medicare, or Medicaid.

You are responsible for confirming whether your insurance provider offers reimbursement for consultations, laboratory testing, prescriptions, or other services.

ConciergeRX does not guarantee insurance coverage or reimbursement. You remain responsible for all applicable fees regardless of whether your insurance provider reimburses you.

8. Health Information and Informed Consent

Before receiving services, you may be required to:

Disclose current medications, supplements, allergies, and health conditions.

10. Prescriptions and Clinical Decisions
